LAS VEGAS (FOX5) -

A Las Vegas preschool's doors were locked Wednesday after the state suspended and revoked the childcare facility's license. The shutdown comes after investigators say workers at Kidz Kidz Kidz, located near the intersection of Valley View Boulevard and Southern Highland Parkway, left a 20-month-old child outside, alone and in triple-digit temperatures.

Kidz Kidz Kidz refused to talk about the incident or the status of its license, but a report from the Nevada Department of Public and Behavioral Health documents the incident.

The report says that at about noon on July 21, one of the teachers at the preschool noticed that one of the children was missing. That teacher looked for the boy outside on the playground, and found the boy unresponsive and dehydrated under the playground equipment. Investigators say the child was alone outside for about an hour and a half.

The report goes on to say the staff at Kidz Kidz Kidz did not call 911 until 16 minutes after the boy was found outside, and didn't contact the parents of the child until 43 minutes after he was found.

The boy was taken to a hospital for observation. His parents said he suffered dehydration and burns on his feet.

Investigators also said that at the time of the incident, there was not a Division-approved director at the preschool.

"The Kidz Kidz Kidz Child Care facility's practices have demonstrated a lack of proficiency for ensuring the safety and well-being of children within their care by violating State regulation and statutes as described," the report reads. "This negligence has resulted in detrimental results of causing harm to children, failure to notify the parents of their child's distress in a timely manner, children not being properly supervised or lack of supervision, and, finally, failure to implement proper emergency protocol by facility leadership, effectively compromising risk and safety of children within their care."

There is no indication on when or if the preschool can re-open. Kidz Kidz Kidz has the right to appeal the ruling.
